At Crossings Cedarmore, they desire for every student to hear the gospel proclaimed, engage with God’s word, and experience Christ-centered worship. They are intentional about deliberately infusing the gospel into every detail of the camp experience. However, meaningful experiences shouldn’t be boring. They desire for campers to make memories they’ll be talking about for the rest of their lives. That’s why they offer a wide variety of activities from zip lines, ropes courses, and water slides to cake decorating, laser tag, and hatchet throwing. Each experience is designed to help our group grow closer together as they grow closer to God in a fun-filled, engaging environment. PARENT/GUARDIAN RELEASE

I, the above listed parent or legal guardian, hereby consent to my child (listed below) participating in Summer Camp at Crossings Cedarmore - an event sponsored by Summit Student Ministries of Henry Baptist Church on May 30-June 3, 2023. I certify that my child is able to participate in the following: lake games, sporting events, bus/van/car transportation, and all other activities conducted to, during, and from camp. I acknowledge the risk of group activities, and I will not hold Henry Baptist Church responsible for any injury or sickness acquired by my child during camp.


If my child has medical conditions which may be relevant to a physician in the event of an emergency, I have listed them below. In the event an emergency occurs, I may be reached at the telephone number(s) given. I hereby authorize any licensed physician, emergency medical technician, hospital or other medical or health care facility to treat the minor named herein for the purpose of attempting to treat or relieve any injury received by said minor. I authorize any such Medical Provider to perform all procedures deemed medically advisable in attempting to treat or relieve any such injuries. I consent to the administration of anesthesia as deemed advisable. I realize and appreciate that there is a possibility of complications and unforeseen consequences in any medical treatment, and I assume any such risk for and on behalf of myself and said minor. I understand that attempts will be made to contact me in the most expeditious way possible. If I cannot be reached, I hereby authorize the adult sponsor, Jon Estes, to make emergency medical decisions for my child. Permission is also granted to Henry Baptist Church representatives to provide the needed emergency treatment to the student prior to his admission to a medical facility. If there are any activities I do not want my child to be involved in, I have listed them below.


I understand and hereby agree to assume all of the risks which may be encountered on said activity, including activities preliminary and subsequent thereto. I do hereby agree to hold Henry Baptist Church and its agents and employees harmless from any and all liability, actions, causes of actions, claims, expenses, and damages on account of injury to my child or property, even injury resulting in death, which my child now has or which may arise in the future in connection with the activity or participation in any other associated activities. I agree not to sue any of the persons or entities mentioned above for any of the claims or liabilities that I have waived, released or discharged herein except in the case of gross negligence on the part of Henry Baptist Church, Henry Baptist Church staff, or volunteers. I indemnify and hold harmless the person or entities mentioned above from any claims made or liabilities assessed against them as a result of my child’s actions. 


I expressly agree that this release, waiver, and indemnity agreement is intended to be broad and inclusive as permitted by the law of the State of Georgia, and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect. This release contains the entire agreement between the parties hereto, and the terms of this release are contractual and not a mere recital.

GENERAL RULES OF CONDUCT

For your information, these are some general rules of conduct expected from each student:

• Respect one another, staff and adult leaders​ 
​ No alcohol, drugs, tobacco permitted 
​ No lighters permitted
​ No fighting, weapons, fireworks, or explosives ​permitted
​ No students permitted to drive for events 
​ Respect the property of Henry Baptist Church and other people
• No offensive or immodest clothing permitted
​ No boys in girl’s sleeping quarter & visa versa
​ Participation with the group expected 
​ Respect and comply with event schedules

Failure to comply with these expectations could result in your child being sent home at your expense.
